Bacillus cereus: Spore-Forming Cause of "Fried Rice Syndrome"
Bacillus cereus is a spore-forming bacterium behind two distinct foodborne illnesses: a fast-acting vomiting syndrome from the heat-stable toxin cereulide (the classic "fried rice syndrome") and a slower diarrheal syndrome from heat-labile enterotoxins. Because its spores survive normal cooking, the danger comes from leaving cooked starchy food at room temperature, where surviving spores germinate and multiply.
Bacillus cereus is a Gram-positive, rod-shaped bacterium found widely in soil, dust, and food. Its defining trait is the ability to form endospores — dormant, heat-resistant cells described in Bacterial Endospores: Dormant Cells That May Survive Millions of Years — which let it persist through ordinary cooking. Boiling, steaming, or stewing at 100 degrees C reliably kills the active vegetative cells but does not destroy the spores; reliable spore destruction takes roughly 121 degrees C for several minutes. As a result the bacterium causes food poisoning not because the food was undercooked, but because it was cooled and stored improperly after cooking. B. cereus produces two clinically distinct illnesses. The emetic syndrome (vomiting type) is an intoxication caused by cereulide, a small cyclic peptide toxin that is preformed in the food before it is eaten. Cereulide is remarkably stable — resistant to heat, acid, and digestive enzymes — so reheating contaminated food does not neutralize it. Symptoms (nausea and vomiting) appear quickly, typically within 1 to 5 hours, and resolve within a day. The diarrheal syndrome is instead an infection: live cells ingested with the food produce heat-labile enterotoxins (such as Hbl, Nhe, and CytK) in the small intestine. Its onset is slower, usually 8 to 16 hours, with watery diarrhea and abdominal cramps that can resemble Clostridium perfringens poisoning. The emetic form is famous as "fried rice syndrome" because cooked rice and pasta are its classic vehicles. When a batch of rice is cooked and then held warm or left on the counter for hours, the spores that survived cooking germinate and the cells churn out cereulide. Cooked starchy foods should be served hot, kept above about 57 degrees C if held, or cooled quickly and refrigerated below 5 degrees C within a couple of hours. Keeping food out of the 4-to-60-degree-C "danger zone" stops spores from germinating, and since cereulide is heat-stable, reheating cannot fix food that has already been left out too long.
