Why Catheters Cause Infections Despite Being Sterile

Catheters are sterile at insertion, but infection risk increases dramatically the longer they remain in place. The problem is not initial sterility but what happens afterward. Mechanisms of catheter-associated infection: 1. Skin bacteria migration: Even with antiseptic preparation, bacteria from the skin surface migrate along the outside of the catheter into the body. The insertion site is a permanent breach in the skin barrier that bacteria can exploit. 2. Biofilm formation: Bacteria adhere to the catheter surface and form biofilms — structured bacterial communities encased in a protective matrix. Biofilms are extremely resistant to antibiotics (up to 1,000x more resistant than free-floating bacteria) and the immune system. 3. Internal contamination: For urinary catheters, bacteria can travel up the inside of the tube. For IV lines, contamination can occur during medication administration or bag changes. 4. Disrupted natural defenses: The catheter bypasses natural barriers (skin, mucous membranes) and can irritate tissue, creating an environment more hospitable to bacterial colonization. This is why hospital protocols mandate removing catheters as soon as they are no longer medically necessary — every additional day of catheterization significantly increases infection risk.
