Larazotide acetate (development codes AT-1001, INN-202) is an eight-amino-acid synthetic peptide that acts as a zonulin receptor antagonist. Zonulin is a human protein that modulates tight junctions between intestinal epithelial cells; elevated zonulin loosens the junctions and is associated with increased intestinal permeability, the mechanism popularly known as 'leaky gut.' Larazotide binds the zonulin receptor and is intended to keep tight junctions sealed. The drug's furthest clinical development is in celiac disease. Innovate Biopharmaceuticals ran a Phase 3 trial called CeDLara that enrolled 525 patients with persistent celiac symptoms despite a gluten-free diet. Enrollment completed in 2022. As of 2026 the trial results remain unpublished and larazotide has not received FDA approval for any indication. Earlier Phase 2 data showed modest symptom improvement. Larazotide has never been tested in IBS, IBD, autism, or any of the other 'leaky gut'-adjacent conditions for which it is marketed. The intestinal permeability hypothesis itself remains contested: permeability changes are observed in many GI conditions but causation versus consequence is unresolved. Like BPC-157, larazotide is sold by research-chemical vendors as lyophilized powder, typically 500mg per oral dose. Buyers have no quality assurance and no human efficacy data outside the unpublished celiac trial. Compounding pharmacies cannot legally produce it because it is not a bulk drug substance on the FDA's approved 503A list. The substance is a legitimate clinical-stage drug candidate, not snake oil, but the gap between 'unpublished Phase 3 in celiac' and 'used as IBS gut-repair stack' is the entire problem.
