Fructose and Uric Acid: Why Sugar Raises Gout Risk
Fructose is unique among common sugars in directly raising uric acid. Its liver metabolism burns through ATP without feedback braking, and the resulting breakdown of AMP cascades down to uric acid, while fructose byproducts also block uric acid excretion. This is the mechanism linking sugary drinks to gout.
Fructose is the dietary component most directly tied to raising blood uric acid, and the mechanism explains why sugar-sweetened beverages carry gout risk comparable to beer and red meat. Fructose is metabolized almost entirely in the liver. The enzyme fructokinase phosphorylates it immediately, consuming ATP rapidly. Crucially, fructokinase lacks the negative feedback that slows glucose-handling enzymes, so it keeps burning ATP as fast as fructose arrives, depleting the cell's energy currency. Falling ATP activates AMP deaminase, which converts AMP to IMP. From there the purine breakdown cascade runs to completion: IMP to inosine to hypoxanthine to xanthine and finally to uric acid. The insult is twofold. Beyond overproducing uric acid, fructose metabolism generates lactate and other organic acids that compete with urate for excretion in the kidney, impairing its clearance. Increased production plus decreased excretion is what makes fructose distinctive among dietary triggers of hyperuricemia.
