Clascoterone (Winlevi): First-in-Class Topical Androgen Receptor Inhibitor for Acne
Clascoterone 1% cream (Winlevi) is the first topical drug to directly block the androgen receptor in skin, FDA-approved in 2020 after Phase 3 trials showed roughly triple the treatment success rate of vehicle — a mechanism previously only available via oral spironolactone or isotretinoin.
Clascoterone (brand name Winlevi) is a topical androgen receptor inhibitor approved by the FDA in August 2020 for acne vulgaris in patients 12 years and older. It is the first topical anti-androgen acne therapy ever approved and the first new acne mechanism approved in roughly 40 years. Clascoterone is a structural analog of dihydrotestosterone (DHT) that binds androgen receptors in sebocytes (sebum-producing cells) and follicular keratinocytes, competitively displacing endogenous DHT and shutting down the local androgen signal that drives sebum production and follicular hyperkeratinization. Because activity is confined to skin (the molecule is rapidly hydrolyzed to an inactive metabolite, cortexolone, on systemic absorption), it avoids the systemic anti-androgen effects of spironolactone and is the only topical that works on the same mechanism. Two identically designed Phase 3 randomized controlled trials enrolled a combined 1,440 patients with moderate-to-severe facial acne, randomized to twice-daily 1% clascoterone cream or vehicle for 12 weeks. The primary endpoint, Investigator's Global Assessment 'treatment success' (clear or almost clear plus a two-grade improvement), was achieved by 18-20% of clascoterone patients versus 6-9% of vehicle — a doubling to tripling of response. Inflammatory lesion and non-inflammatory lesion counts dropped significantly more in the active arm. Adverse events were mild and similar to vehicle. The practical significance: clascoterone gives men a topical anti-androgen for the first time. Spironolactone, the standard systemic anti-androgen for women's acne, is generally avoided in men because of breast tissue, libido, and electrolyte effects. Combination protocols pair clascoterone with a retinoid (tretinoin or tazarotene) and an antimicrobial, hitting androgen signaling, keratinization, and bacterial overgrowth simultaneously.
