Cetylpyridinium chloride is a quaternary-ammonium cationic surfactant used in many alcohol-free mouthwashes, toothpastes and lozenges. Cetylpyridinium chloride (CPC, formula C₂₁H₃₈ClN) is a quaternary ammonium compound that acts as a cationic surfactant and antiseptic. It is the active antibacterial in many alcohol-free daily mouthwashes (for example gum-health formulations) and also appears in toothpastes, lozenges, throat sprays and nasal sprays. **Mechanism.** As a positively charged surfactant, CPC is attracted to negatively charged bacterial surfaces, where it disrupts the cell membrane and kills the organism. The same detergent action can break the lipid envelopes of certain enveloped viruses. **Efficacy.** Reviews find that CPC-containing rinses give a small but statistically significant additional reduction in dental plaque and gingivitis when added to toothbrushing alone. Its antiplaque effect is meaningfully weaker than chlorhexidine, but it is gentler and better suited to long-term daily use. Combined with chlorhexidine and zinc lactate it is also used against halitosis (bad breath). **Safety and dosing.** Regulators have concluded that 0.025-0.1% CPC is safe as an over-the-counter oral antiseptic when labeled for short-term use; rinses are commonly formulated around 0.05%.
